# Does Tretinoin Help Acne Scars? What It Can and Can't Do

> How tretinoin affects acne scars and post-acne marks: what it improves (texture, dark marks, new breakouts), its limits for deep scars, and how to combine it with procedures.

Tretinoin helps acne scarring indirectly. The short answer: it can fade dark marks, smooth shallow texture over months and, most importantly, prevent new acne and new scars. It won't fill deep icepick, boxcar or rolling scars; those need procedures such as microneedling, lasers or subcision, which tretinoin can complement.

## What tretinoin does

- **Treats acne:** the AAD guidelines recommend topical retinoids for acne ([Reynolds et al., *J Am Acad Dermatol*, 2024](https://pubmed.ncbi.nlm.nih.gov/38300170/)).
- **Speeds cell turnover:** helps fade post-inflammatory hyperpigmentation.
- **Improves texture:** tretinoin improved photoaged skin in a controlled trial ([Weiss et al., *JAMA*, 1988](https://pubmed.ncbi.nlm.nih.gov/3336176/)), which supports its use for overall texture.

## Marks vs scars

| | Responds to tretinoin? |
|---|---|
| Brown marks (PIH) | Often, over months |
| Red marks (PIE) | Limited; vascular laser helps more |
| Shallow texture | Some improvement |
| Icepick, boxcar, rolling scars | Minimal; need procedures |
| Raised scars | No; need steroid injections or laser |

Small older studies tested tretinoin delivered by iontophoresis (an electrical current) for atrophic acne scars and reported improvement ([Schmidt et al., *Int J Dermatol*, 1999](https://pubmed.ncbi.nlm.nih.gov/10192170/)), but that's different from applying cream at home.

## Combining with procedures

- **Microneedling** has randomized-trial support for atrophic acne scars ([Sitohang et al., *Int Wound J*, 2021](https://pubmed.ncbi.nlm.nih.gov/33538106/)).
- **Lasers, TCA CROSS and subcision** treat specific scar types.
- Stop tretinoin before procedures as your provider advises, then restart after healing.

## How to use tretinoin

1. Start two or three nights a week with a pea-sized amount.
2. Moisturize.
3. Increase as tolerated.
4. Use sunscreen daily ([AAD: sunscreen FAQs](https://www.aad.org/public/everyday-care/sun-protection/shade-clothing-sunscreen/sunscreen-faqs)).
5. Avoid during pregnancy.

## Purging

Tretinoin can cause an initial flare of breakouts for a few weeks as it speeds turnover. Persistent worsening should be discussed with your doctor.

## Darker skin

Irritation can cause dark marks. Start slowly and moisturize ([Davis and Callender, *J Clin Aesthet Dermatol*, 2010](https://pubmed.ncbi.nlm.nih.gov/20725554/)).

## Tretinoin as a partner for procedures

A 2023 review of topical acne scar treatments says retinoids have long been used to prevent and treat scars, and that procedures such as microneedling and ablative lasers are often combined with topicals. Iontophoresis, which uses a mild electric current, helped deliver tretinoin into scarred skin. The authors also stressed that there isn't enough good evidence to support widespread use of topicals for scars alone and called for better studies ([Bikash and Sarkar, *J Cosmet Dermatol*, 2023](https://pubmed.ncbi.nlm.nih.gov/36606377/)).

In practice, that means tretinoin is best seen as long-term maintenance that improves texture and helps prevent new acne (and new scars), while microneedling, lasers or subcision do most of the work on established scars.

## Frequently asked questions

**How long does tretinoin take for marks?** Often three to six months.

**Can tretinoin make scars worse?** Not directly, but irritation can darken marks in some skin types.

**Is adapalene similar?** It's an OTC retinoid for acne with similar principles.
