Proper aftercare following microneedling is critical because the thousands of micro-channels created during treatment are open gateways — they allow beneficial serums to penetrate deeply, but they also expose the dermis to potential infection and irritation if the wrong products or practices are used. Following a structured aftercare protocol maximizes results while minimizing complications. Recovery is typically 24-72 hours of redness followed by return to normal appearance.


Immediately After Treatment (Hours 0-6)

The skin will appear red and feel warm, like a moderate sunburn. Mild swelling is normal, especially around the eyes and cheeks. Apply only the serum or product provided by your practitioner — do not apply anything from your home collection during this window. Avoid touching your face with unwashed hands. Do not apply makeup. Avoid direct sunlight. Do not exercise or engage in any activity that causes sweating. Do not consume alcohol. Sleep on a clean pillowcase — ideally silk or satin, which creates less friction. Keep the room cool to minimize sweating during sleep. If mild bleeding occurred during treatment, it typically stops within one to two hours.

Day 1 After Treatment

Redness resembles a moderate sunburn and begins to fade. Cleanse gently with lukewarm water and a very gentle cleanser (micellar water or Cetaphil Gentle). Apply a hyaluronic acid serum followed by a simple, fragrance-free moisturizer (CeraVe, Vanicream). Apply mineral sunscreen if going outdoors. Continue avoiding: makeup, active ingredients (retinoids, vitamin C, AHAs, BHAs, benzoyl peroxide), exfoliants of any type, swimming pools, hot tubs, saunas, and intense exercise. The micro-channels are closing but the skin barrier is not yet fully restored.

Days 2-3 After Treatment

Redness continues to fade. Some patients experience mild dryness and tightness as the surface heals. A gentle vitamin C serum (ascorbic acid 10-15% or sodium ascorbyl phosphate) can be introduced on day two if tolerated. Continue gentle cleansing, hyaluronic acid, moisturizer, and sunscreen. Mineral makeup can typically be applied on day two if needed. Light exercise can resume on day two with thorough face cleansing immediately after. Most patients appear normal by end of day two to three. Patients with deeper treatment (1.5mm+) may have lingering redness and slight roughness through day three.

Days 4-7 and Beyond

By day four, the skin barrier is essentially restored and normal skincare can be resumed gradually. Reintroduce products one at a time: retinoid on day five to seven, exfoliating acids on day seven. The skin may feel unusually smooth and have a subtle glow as new collagen production begins. Deeper hydration is common as the micro-channels have enhanced product absorption capacity. Continue diligent sunscreen for two weeks minimum. If any product causes unusual stinging or redness, wait another two to three days before retrying.

What to avoid and why?

Avoid retinoids for 48-72 hours because they can cause excessive irritation on compromised barrier skin. Avoid chemical exfoliants for five to seven days as they interfere with the controlled healing process. Avoid makeup for 12-24 hours because cosmetic pigments and chemicals can enter micro-channels and cause irritation or granuloma formation. Avoid direct sun for two weeks to prevent hyperpigmentation on sensitized skin. Avoid swimming pools and hot tubs for 72 hours due to bacterial contamination risk in open micro-channels. Avoid intense exercise for 24-48 hours because sweat contains bacteria and salt that can irritate treated skin.

Can I wash my face after microneedling?

Wait at least six hours after treatment before washing. When you do cleanse, use lukewarm (not hot) water and the gentlest cleanser available — micellar water or a creamy non-foaming cleanser. Pat dry with a clean towel, do not rub. Avoid any cleanser containing acids, fragrance, or physical exfoliating particles for at least 48 hours.

When can I wear makeup after microneedling?

Most practitioners recommend waiting 12-24 hours. Mineral makeup is preferred when first resuming because it is less likely to irritate healing skin. Conventional makeup with chemical ingredients and heavy pigments should be avoided for 48 hours. Ensure all brushes and applicators are freshly cleaned before applying to post-microneedling skin.

What if I break out after microneedling?

Small breakouts one to three days after microneedling can occur due to the skin purging (accelerated shedding of comedones from the micro-channeling effect) or from product occlusion during the healing phase. This typically resolves within five to seven days. If breakouts are severe, persistent, or accompanied by pain and swelling, contact your provider as this could indicate infection requiring treatment.
